		BACK Enrolment To assist with the process you will need to provide the school with originals of the following documents. These will be photocopied (if required) and returned immediately. Proof of residence must be confirmed by providing two of the following. This documentation needs to be presented at the same time as the enrolment form. Current rental lease for at least 6 months tenure Current council rates notice/payment Current telephone account/payment Current water rates account/payment Current electricity account/payment The Department of Education and Training requires the school to sight originals of the following: Birth Certificate and / or passport Immunization History for your child (automatically received after booster shots) a copy is available by phoning 1800 653 809 or a written statement indicating an objection to immunizations. If applicable, passports of both parents Proof of residence (as explained above) Any documents regarding guardianship or access The Strathfield North Public School documents: School Census Form Medical Information form Permission to Publish Internet Code of Conduct A selection panel will review Non Local enrolments. Enrolments will be considered on the criteria below (not listed in priority order) Availability of appropriate staff and permanent classroom accommodation Sibling currently enrolled at the school Proximity and access to the school Safety and supervision of students before and after school Medical reasons Special interest and abilities Compassionate circumstances Structure and organisation of the school No child will be discriminated against for enrolment because of gender, ethnicity, religion and / disability. Waiting Lists A waiting list will be established for no-local students. Parents will be advised in writing if their child is placed on a waiting list. All waiting lists will be current for one year only.
